SCHEDULE A - SINGLE PHASE SERVICE
INSTALLATION ONLY
AVAILABILITY
Available to any member of the Corporation located in service area for General Electric Service that can be adequately served with single-phase electric power. All service shall be delivered through a single set of service wires at a single service location, the service location to be designated by the Corporation.
All applications for service shall be in accordance with the provisions of the Corporation Standard Application Form and in accordance with the provisions set forth the in this Schedule, and also in accordance with the Corporation's General Rules.
CHARACTER OF SERVICE:
Type of service rendered under this Schedule shall be alternating current, sixty-cycle, single-phase, at a voltage of 120 volts, two-wire, 120/240 three-wire, and not to exceed the limitations as set forth in the Rules and Regulations of the Indiana Utility Regulatory Commission.

RATE ADJUSTMENT:
The above rates are subject to a Purchased Power Cost Adjustment Tracking Factor. The Purchased Power Cost Adjustment Tracking Factor stated in Appendix A is applicable hereto and is issued and effective at the dates shown on Appendix A.
FUEL COST ADJUSTMENT:
Refer to Appendix A.
MONTHLY MINIMUM CHARGE:
The Monthly Minimum Charge under the above rates shall be not less than $15.00 plus any other adjustments that may be in effect and subject to the aforementioned "Terms and Conditions Governing Extension of Single-Phase Service" as hereinafter provided.
TERMS OF PAYMENT:
Bills for service rendered under this schedule shall be due and payable monthly. If paid within seventeen days from the due date thereof as stated in the bill, the net bill shall be the amount to be paid. If not paid on or before the seventeenth day following the due date, the gross bill, which shall include the late payment charge, shall be the amount to be paid. When the seventeenth day falls on Sunday or a legal holiday, the first business day thereafter shall be added to the seventeenth day net payment period. The U.S. Postmark on mailed remittances will be used as the day of payment.
